Description: Usage :It is a HMGCoA reductase inhibitor. Mechanism of Action :It acts by competitively inhibiting 3-hydroxy-3-methyl glutaryl co-enzyme A (HMGCoA) reductase, an enzyme involved in cholesterol synthesis. The greater efficacy of atorvastatin appears to reflect the longer residence time of the drug in the liver or its active metabolites which results in more prolonged inhibition of HMGCoA reductase than that occurs with other statins. Pharmaco Kinetics :It is rapidly absorbed after oral administration. It exhibits hepatic first pass metabolism. It is extensively bound to plasma proteins. It is almost completely metabolised and its metabolites are eliminated primarily in the bile. Adverse Effects :Abdominal pain, nausea and vomiting. Reversible myositis, headache, altered LFTs, insomnia, anorexia, alopecia, impotence, chest pain, hypoglycaemia and hyperglycaemia. Special Precautions :Patients should be advised to report promptly unexplained muscle pain, tenderness or weakness, particularly if accompanied by malaise or fever (stop therapy if markedly elevated creatinine phosphokinase levels occur). Pregnancy :Contraindicated. Breast Feeding :Contraindicated. Old Age :May be used. Interactions :Antacids: Absorption of atorvastatin decreased but no effect on LDL-C reduction. Colestipol: Plasma concentrations of atorvastatin decreased. However LDL-C reduction is greater. Digoxin: Slight elevation in digoxin levels. Erythromycin: Levels of atorvastatin increased by 40%. Indications :Primary hypercholesterolaemia, heterozygous familial hypercholesterolaemia, homozygous familial hypercholesterolaemia or combined hyperlipidaemia. Dosage :Initially 10mg daily.
